Waiver of Liability Form – Registration
This form is to be signed by the legal guardian and delivered to the captain upon registration and every year at re-registration.
Chaperone Form
This form is to be used when the legal guardian is not available to come to a specific TSG event or camp, and the guard of the child is delegated to someone else than the legal guardian. This is a one-time form and cannot be used as a power of attorney.
NOTE: A chaperoned cadet is not allowed to spend the night at TSG-sponsored events or camps.

Power of Attorney Form
This form is to be used once and for all, in the situation where the cadet(s)’s father is either not present in his life or does not wish to have any involvement with the troops, but the mother/legal guardian does. Examples of a guardian could be brother, uncle, grandfather, godfather, etc… This assigned person will act in lieu of the father, without restrictions. This form is to be filled out by the child’s legal guardian and will be kept on file by the TSG1 captain until the legal guardian wishes to remove this permission.

Please understand the spirit of this form in light of the TSG mission statement. This is in no way, shape, or form, a way to “dump” one’s son(s) off for the weekend. TSG is decidedly a father/son organization.
Camp Waiver of Liability Form – Captain use
This form is for the captain or highest present officer to bring to every event. Every father or guardian is to sign it upon arrival.
TSG1 Blanket Health & Accident Insurance Policy
TSG members are covered by this Blanket Health & Accident Policy during TSG1-sponsored events. This policy has been absorbed by TSG National, and is one of the expenses your TSG annual dues cover.
